To access your form template, click the 'New form' button on the Forms page.

Next, select the 'Your saved templates' tab to use access your template.

Then, click the 3-dot menu next to your template to manage your form. Here you'll be able to rename or delete your form.

Note: form templates cannot be recovered once they have been deleted.
